Applies to MAT/MAED/Rank I/administrative certifications (except Principalship)
Specific Praxis requirements
MAT
In order to submit your rank change and/or certification to the Standards Board, you need to complete the following:
Access the following link to complete the Transcript Request Form. This is required for the Certification Office to obtain a copy of your official transcript (Univ Cumberlands) to complete your TC1. Please request this transcript be sent to S. Jones - Education Department. If you are currently enrolled in coursework, please note that the request needs 'held for current semester grades'. Upon completion of an MAT degree, you receive one (1) free official transcript. If you want to use this free transcript, complete the request form, print (from your browser) and write 'Use free transcript' across the top of the request form. You would then mail this form to the Certification Office along with your TC1.
An official transcript cannot be released if you have an account balance with the University.
If this is your initial teaching license, you will also need to have an official copy of your Bachelors transcript sent directly to EPSB (their address is located on the top of the TC1 application).
Access the following link to download the TC1 application. Complete page 1, page 3 and the top portion of page 5 (do not complete section 4). The completed application must be mailed to the Certification Office – Attn: Sherryll Jones.
TC1 Application
Superintendent's signature at the bottom of page 1 is NOT required.
Submit your certification fee to EPSB electronically at www.kyepsb.net. Initial licensure candidates are not assessed a fee.
Once your application is processed and forwarded to EPSB, you may track the status of your application at www.kyepsb.net.
MAED/Rank I/Administrative Certifications

Principal certification
Applies to MAED Instructional Leadership and Principal administrative certification
In order to submit Principal Certification to Kentucky EPSB, candidates are required to have passing scores on the KYPT - Kentucky Principal Test and SLLA - School Leaders Licensure Assessment. Candidates must have an official copy of their SLLA and KYPT principal assessment scores sent directly to UC from ETS/Praxis. Below is a link to the Standard Board, Principal assessment requirements. Candidates are required to have a minimum of three years teaching experience in order to submit Principal certification.

Please note that EPSB has called for a statewide re-design of the Principal program. Any candidate who admitted to a Principal program prior to December 31, 2011, must submit Principal certification to EPSB by January 31, 2014.

Temporary Provisional license
Our Master of Arts in Teaching (MAT) degree program is an alternative route certification program which is eligible for Option 6/Temporary Provisional license. Once a candidate is fully admitted to the institution and admitted to Teacher Education, they are eligible to seek a teaching position in the area in which they are pursuing teacher certification. When hired, a candidate is then recommended for a Temporary Provisional license. Once the TC-TP (Temporary Provisional) application is completed by the hiring school district and the candidate, the complete application (pg 1,3,4) must be mailed to the Certification Office – Attn: Sherryll Jones.
Requirements for a Temporary Provisional license:
When a candidate teaching under Temporary Provisional license completes their degree program, they submit as an MAT completer unless they are currently participating in KTIP. Any candidate who completes the degree program while participating in KTIP needs to contact Sherryll Jones in the Certification Office for submission requirements.
Regular Five Year Renewal
Candidates who need to apply for a standard Five Year renewal must complete the TC-2 and submit directly to EPSB. This application may be found on the EPSB website.
